Application checklist

What do I need to bring?

General

• Each adult, 18 years or older, household member (need to be present at time of application)

• $25 money order for each adult in the household

• Two forms of ID (Driver’s License/State ID, Passport, birth certificate and Social Security Card)

• Social Security cards for ALL household members (including children)

• Names, phone, fax, email for all employers and financial institutions

Employment & Income

• First paystub of fiscal year + last 4 paystubs (all pages and no summaries)

• Self-employed: 1040 Schedule C, 4506-T, OR Profit & Loss (4–6 months)

Other Income (if applicable)

• Child Support: 12-month history

• Unemployment/TANF: 12-month history

• Social Security: Annual award letter or current letter showing benefits

• SSI/SSDI: current letter showing benefits

• Pensions/Other income: documentation required

Section 8 (if applicable)

• Request for Tenancy Approval (obtain from your caseworker)

Prior Landlord Information

• Last 5 years of Landlord Information (name, number, email)
